Monthly Membership
Pricing
- Adults (16+ years old): $75/month
- Children (15 & younger): $35/month (Must be enrolled with an active adult member, covers acute needs only.)
- Initial Evaluation: $90 due at time of services, to include physical and routine lab panel: CBC, CMP, Lipid, A1C, and TSH (Labs are Optional)
